الوظائف التنفيذية لدى الأطفال ذوي متلازمة داون وعلاقتها بالفص الجبهي

ملخص الدراسة
مقدمة
هدفت الدراسة إلى تقييم كفاءة أداء الوظائف التنفيذية ( الذاكرة العاملة , والقدرة على كف الاستجابة , والقدرة على التخطيط , والقدرة على الانتباه المتواصل ) , ومستواه عند الأطفال ذوي متلازمة داون, مقارنة بعينة من الأطفال الأسوياء المطابقين لهم في العمر العقلي . ووضع صفحة نفسية معرفية لمكونات الوظائف التنفيذية عند الأطفال ذوي متلازمة داون. والتحقق من كفاءة وظائف الفص الجبهي للدماغ عندهم , وعلاقته بكفاءة أداء الوظائف التنفيذية من خلال قياس قوة تردد موجات بيتا , والتحقق من وجود لا تماثل بالفص الجبهي بين نصفي الدماغ في قوة تردد موجات بيتا.
مشكلة الدراسة
1- هل توجد فروق جوهرية بين الأطفال ذوي متلازمة داون مقارنة بالأطفال الأسوياء المكافئين لهم في العمر العقلي في كفاءة أداء الوظائف التنفيذية الخاصة بالفص الجبهي ومستوى هذا الأداء ؟
2- هل يوجد ارتباط بين أداء مهام مكونات الوظائف التنفيذية, ونشاط الفص الجبهي للدماغ ؟
3- هل توجد فروق في كفاءة أداء الفص الجبهي للدماغ لدي الأطفال ذوي متلازمة داون مقارنة بالأطفال المطابقين لهم في العمر العقلي ؟
4- هل توجد فروق في قوة انتشار موجات بيتا بين الفص الجبهي الأيمن , والفص الجبهي الأيسر لدى حالة الطفل ذي متلازمة داون مقارنة بحالة الطفل السوي في أثناء الأداء على اختبارات الوظائف التنفيذية ؟
5- هل تختلف الصفحة النفسية المعرفية لمكونات الوظائف التنفيذية لدى الأطفال ذوي متلازمة داون عن الأطفال الأسوياء المكافئين لهم في العمر العقلي ؟
عينة الدراسة
1- مجموعة الحالة : تكونت من 20 طفلًا ذي متلازمة داون (15 ذكرًا , 5 إناث ), وتراوح عمرهم العقلي 3.3سنوات حتى 6.3 سنوات .
2- مجموعة المقارنة: تكونت من 20 طفلًا سويًا (15 ذكرًا , 5 إناث ), وتراوح عمرهم العقلي 3.3سنوات حتى 6.3 سنوات , مكافئين لأطفال مجموعة الحالة في العمر العقلي.
3- مجموعة تجربة رسم الدماغ : تكونت من طفل ذي متلازمة داون , وطفلة سوية (حالة المقارنة) مكافئة للطفل ذي متلازمة داون في العمر العقلي.
الأدوات:
استخدم اختبار رسم الرجل , وبطارية لتقييم الوظائف التنفيذية من إعداد الباحث ؛اختبار التعرف البصري المكاني للذاكرة العاملة , واختبار ستروب الليل-النهار, واختبار المتاهات الخشبية , واختبار القدرة على الانتباه المتواصل. وجهاز رسام الدماغ الكهربائي.
الاساليب الإحصائية :
وتمثلت في: معامل الارتباط بيرسون , و معاملات الالتواء والتفلطح, و المتوسطات الحسابية والانحراف المعياري , و اختبار مان- ويتني (Mann-Whitney ) لدلالة الفروق.
النتائج:
- أظهرت نتائج الدراسة أن هناك فروق دالة في كفاءة أداء الوظائف التنفيذية بين الأطفال ذوي متلازمة داون , والأطفال الأسوياء المطابقين لهم في العمر العقلي لصالح الأطفال الأسوياء, ويقع مستوى أداء الأطفال ذوي متلازمة داون للوظائف التنفيذية موضع اهتمام الدراسة في مستوى أداء أقل من العمر العقلي لهم. بينما لم يكن هناك فروقًا دالة في القدرة على التخطيط.
- وجد أن هناك علاقة دالة بين أداء الوظائف التنفيذية وبين النشاط في الفص الجبهي, متمثلة في زيادة قوة تردد موجات بيتا.
- أن هناك فروق دالة بين حالة الطفل ذي متلازمة داون وحالة الطفل السوي في كفاءة أداء الفص الجبهي , لصالح الطفل السوي.
- تتسم الصفحة النفسية المعرفية للأطفال ذوي متلازمة داون بالضعف في أداء الذاكرة العاملة , والقدرة على كف الاستجابة, ويعانون من ضعفًا نسبيًا في القدرة على التخطيط, كما يعانون من عدم القدرة على تركيز واستمرار الانتباه على منبه ما. بينما يظهرون تحسناً نسبياً في القدرة على التذكر البصري المكاني كأحد مكونات الذاكرة العاملة.

هدفت الدراسة الراهنة إلى تحديد الفروق التشخيصية للأداء النفسي العصبي المعرفي الواسم لمرض اضطراب التوحد وذلك من خلال وضع صفحة نفسية عصبية معرفية باستخدام اختبار وكسلر لذكاء المراهقين والراشدين: التعديل الرابع- كأداة للفرز النفسي العصبي، ومرضى الفصام غير الهذائي في مقابل تحديد الخصائص الواسمة للصفحة النفسية المعرفية للأسوياء، وتحديد نقاط القوة والضعف في الأداء الوظيفي المعرفي، وتحديد كفاءة أداء الوظائف المعرفية المقاسة بالمقاييس الفرعية لاختبار وكسلر لذكاء المراهقين والراشدين التعديل الرابع. وتكونت عينة الدراسة من 34 مريضًا بالفصام، و34 مريضًا بالتوحد، و34 مشاركًا من الأسوياء، تراوح عمرهم من 16 سنة إلى 21 سنة. وقد وجدت النتائج أن هناك نمطًا مميزًا للصفحة النفسية العصبية المعرفية واسمًا لمرضى الفصام غير الهذائي ومرضى التوحد مقارنة بالأسوياء، كما وجد أن هناك تباينًا بين المجموعات الثلاث في الأداء على المقاييس الفرعية ودرجة المكونات الأربعة والدرجة الكلية لاختبار وكسلر وفقًا للمرض. وأيضًا وجد فروق ذات دلالة إحصائية في كفاءة أداء الوظائف العصبية المعرفية لدى مجموعات الدراسة الثلاث.
